3 Waiver & Release

1. Assumption of Risk

I acknowledge that basketball and related athletic activities involve inherent risks of physical injury. I voluntarily assume all risks associated with participation in Westside Kings & Queens programs.

2. Liability Release

I release and hold harmless Westside Kings & Queens, its owners, coaches, staff, and volunteers from any and all claims arising from participation, except in cases of gross negligence.

3. Medical Authorization

In the event of a medical emergency, I authorize coaching staff to seek emergency medical treatment if I or my emergency contact cannot be reached.

4. Photo/Video Release

I grant permission to photograph and record the player during program activities for social media, marketing, and program documentation.

5. Code of Conduct

I agree that the player will demonstrate good sportsmanship, respect coaches and officials, and attend regularly. Westside reserves the right to dismiss any participant whose behavior is detrimental to the program.
